New Patient Packet
Save time in the waiting room! Please print and complete the following forms before your appointment and remember to bring them with you to your appointment. These forms should be used for all new patients and those established patients who have not been to our offices in the past three years.
Please complete the forms below if you have not been seen in our office since March 1, 2015. Also, please be prepared to show your ID and insurance card for our new records system.

If you don't see your plan listed, please contact our office. The managed care health insurance market changes rapidly, and specific coverage questions should be discussed directly with your insurance company or our knowledgeable staff.

Please include reason for referral, pertinent imaging studies, and insurance information.
Co-pays are due at the time of your office visit. For your convenience, we accept MasterCard and Visa, as well as personal checks and cash.
For billing records or questions, please contact our billing company directly at (858) 513-1833. Press extension #127 for Dr. Peppers' billing and extension #120 for Dr. Glenn's billing.
Please arrive 15 minutes prior to your initial appointment and bring your valid insurance card or HMO authorization notice. If you've had recent diagnostic studies performed (i.e., X-rays or MRI services), please bring them with you to help expedite your care. Additionally, if you have any written reports from other physicians, please bring them to your appointment.

No Show and Cancellation Policy
If you wish to cancel your appointment, please do so 24 hours prior to your scheduled appointment. If you fail an appointment or do not cancel 24 hours in advance, you will be billed a "no-show" fee.
It is the office policy that medication refills be handled during the hours of 8:30 a.m. to 4:30 p.m. Monday through Thursday. Please allow 48-hours for your request(s) to be completed. Prescriptions cannot be filled over the weekend.
